What Is the Process of Acquiring Home Health?
Silver Star Home Health works by referral from a Physician, Physician Assistant, or Nurse Practitioner. These referrals can come through a doctor’s office, local hospital, skilled nursing facility, or residential care facility. Clients, families, or caretakers can inquire, and we can contact your physician directly. Once we receive a referral, our clinical team reviews the patient’s anticipated needs and our current ability to safely provide the required services. If the referral is accepted, we coordinate the start of care within the required and mutually appropriate time frame.

Some specialty services or high-frequency care needs may have limited availability. Before accepting a patient for care, Silver Star Home Health reviews the referral and anticipated care needs to determine whether we have the appropriate resources and qualified personnel available to safely meet those needs.
Acceptance of a referral is therefore determined on an individual basis and is not guaranteed solely because a service is generally offered by the agency.
If we are unable to safely provide a requested service or meet the required frequency, duration, specialty-care need, or start-of-care timeframe, we will communicate this to the referral source or prospective patient as appropriate.
